Below is a sample Client Information form for use for “first time” clients. Using this form will allow you to look more professional, get a better picture of your new client, and begin to develop that key relationship that is so important for service based businesses.

• disclaimer – Client understands that you are dealing with animals. Again, this helps to protect you against spurious claims
